MH Services and Supports Coordination

Supports Coordinators (previously known as Case Managers) are human service professionals employed by the Base Service units or the Bucks County Department of Mental Health/Development Programs. Supports Coordinators work with individuals with mental retardation. Supports Coordinators formulate plans to provide various supports and services according to individual need. Supports Coordinators act as liaisons between the individual, family, service providers and the community. The primary functions of Supports Coordinators are assessment and assistance in locating supports in the community. They collaborate with other agencies, facilitate and coordinate the development of service plans, and provide support and advocacy in addition to monitoring service delivery.

The directory can help you locate particular service providers or search for services and supports provided in an individual's community. This directory also provides information about specific services funded by the Office of Developmental Programs (ODP) under the home and community-based waiver programs
